Eastern Melbourne PHN sought to build an integrated 'Service Directory' (replacing its SupportConnect directory) underpinning its GP mental-health model (Link-Me triage, Care Navigators) -- helping GPs, navigators and consumers find and refer to local health and social-support services, including social prescribing, with a focus on accessibility for CALD, homeless and other hard-to-reach populations across 12 local government areas.
