Abstract
Transitions—from starting school to moving into employment—can profoundly shape a learner‘s experience. In this chapter, we explore the theoretical and practical sides of educational transitions, highlighting how inclusive practices and person-centered planning can empower students at every stage. Through stories and strategies, discover how resilience, equity, and a collaborative mindset help learners—especially those from marginalized backgrounds—navigate these pivotal shifts successfully.
